By accessing or using HubMD (“the Platform”), you agree to be bound by these Terms of Service (“Terms”). If you do not agree to all of the terms and conditions set forth herein, you may not access or use the Platform. These Terms constitute a legally binding agreement between you and HubMD.

Important Medical Disclaimer: The Platform facilitates communication between healthcare providers and patients. It does not replace professional medical advice, diagnosis, or treatment.
Always seek the advice of your physician or other qualified healthcare provider with any questions you may have regarding a medical condition. Never disregard professional medical advice or delay in seeking it because of something you accessed through the Platform.
HubMD provides technology infrastructure to support healthcare delivery, including secure messaging, appointment management, prescription tracking, and electronic health records. The healthcare services themselves are provided by licensed healthcare professionals who are responsible for their own clinical decisions.
By using the Platform, you agree to receive communications from HubMD, including:
Appointment summaries, lab results, account updates, and important care notifications.
Appointment reminders, prescription updates, and important healthcare notifications (with your consent).
Real-time updates within the Platform about your care, messages from your provider, and account activity.
You may manage your communication preferences at any time from your Account Settings. See our Messaging Consent Practices for details on how SMS consent is collected and honored.
If applicable, you agree to pay all fees associated with your use of the Platform. All billing information is processed through secure, PCI-compliant payment processors. HubMD does not directly store credit card numbers. Payment terms, billing cycles, and refund policies may vary by service and will be disclosed before any charges are incurred.
Your use of the Platform is also governed by our Notice of Privacy Practices. HubMD is designed to meet the requirements of the Health Insurance Portability and Accountability Act (HIPAA) for the protection of Protected Health Information (PHI). Please review our Privacy Practices to understand how your health information is collected, used, disclosed, and safeguarded.
The Platform and its original content, features, and functionality are owned by HubMD and are protected by copyright, trademark, and other intellectual property laws. You may not reproduce, distribute, modify, or create derivative works of any portion of the Platform without our prior written consent.
We reserve the right to modify these Terms at any time. We will notify you of material changes through the Platform, via email, or by posting the updated Terms with a revised “Last Updated” date. Your continued use of the Platform after changes are posted constitutes your acceptance of the modified Terms. If you do not agree with the updated Terms, you must stop using the Platform.
We may terminate or suspend your account at any time, without prior notice, for conduct that we determine violates these Terms, is harmful to other users, or is otherwise objectionable. You may terminate your account at any time by contacting us.
Upon termination, your right to use the Platform will cease immediately. We will retain your health information as required by HIPAA and applicable state laws (minimum 6 years after the last date of service). You may request a copy of your health records in accordance with your rights under our Notice of Privacy Practices.
To the maximum extent permitted by applicable law:
These Terms shall be governed by and construed in accordance with the laws of the State in which HubMD is incorporated, without regard to its conflict of law provisions. Any disputes arising under these Terms shall be resolved through binding arbitration in accordance with the rules of the American Arbitration Association, except where prohibited by law. Nothing in these Terms limits your rights under HIPAA or other applicable healthcare regulations.
